If you are using this form to submit a grievance please be sure to include all relevant facts in the space provided.  It is not limited to or mandatory to use this form to submit a grievance request for grievance.  It is important to recognize that the grievance chair will review and present to the grievance committee members to discuss.  It will then be based on the determination of the committee as to whether or not it will move forward.  It is the role of the executive to take forward grievances, not individual members.

It is also important to recognize that the executive board should be notified immediately of a possible violation of the collective agreement, OHSA, Human Rights, Employment Standards etc.
